- A written application addressed to the Director of CISMED-SANTE
- A completed registration form (available at CISMED-SANTE)
- A photocopy of the diploma, transcript, or certificate of success for the GCE A-Level, or any equivalent qualification (for 3-years Bachelor program)
- A photocopy of the diploma, transcript, or certificate of success for the HND, or any equivalent health-related diploma (for 1-year Bachelor program)
- A photocopy of the birth certificate
- A photocopy of the National Identity Card
- Two recent passport-size photos (4x4 cm, taken within the last 3 months)
- One A4-sized envelope
- One box of 5 reams of high-quality A4 paper (80g)
